Your Health Information, Your Rights
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
At Renovance Health, we understand that your health information is personal. We are committed to protecting your Protected Health Information (PHI) in compliance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable New Jersey state laws. We are required by law to maintain the privacy of your PHI and to provide you with this notice of our legal duties and privacy practices.
How We May Use and Disclose Your Health Information
We use and disclose your PHI for the following purposes:
Treatment: We use and disclose your PHI to provide, coordinate, or manage your healthcare. This includes communication with pharmacies, laboratories, and other healthcare providers involved in your care.
Payment: We use and disclose your PHI to bill and collect payment for services rendered to you.
Healthcare Operations: We use and disclose your PHI for internal operations, including quality assessment, staff training, licensing, and business management.
Business Associates: We may disclose PHI to third-party vendors (e.g., billing services, telehealth platforms, IT providers) that perform services on our behalf, provided they have signed a HIPAA-compliant Business Associate Agreement.
Permitted Disclosures Without Authorization
In accordance with law, we may disclose your information without your consent in specific situations, including:
Legal Proceedings: As required by court order, subpoena, or applicable law.
Public Health & Oversight: For health oversight activities or reporting requirements mandated by state or federal agencies.
Safety & Harm Prevention: We are legally obligated to report suspected abuse or neglect of children or vulnerable adults to the New Jersey Abuse Hotline. We may also disclose information if we believe there is a clear and immediate probability of physical harm to you, others, or society.
Defensive Use: If you file a complaint or lawsuit against us, we may disclose relevant information to defend our practice.
Your Patient Rights
You have the following rights regarding your health information:
Right to Access & Copy: You have the right to inspect or obtain a copy of your PHI. Requests must be in writing. In accordance with New Jersey law, we will process your request within 14 days. Reasonable fees may apply.
Right to Amend: If you believe your records are incorrect or incomplete, you may request an amendment in writing.
Right to Request Restrictions: You may request restrictions on certain uses and disclosures of your PHI, though we are not required to agree to all requests (except as required by law for services paid in full out-of-pocket).
Right to Confidential Communications: You may request to receive communications by alternative means or at alternative locations.
Right to an Accounting: You have the right to request an accounting of certain disclosures of your PHI made by our practice.
Right to a Copy of This Notice: You are entitled to a paper or electronic copy of this notice at any time.
Breach Notification
In the event of a breach of your unsecured PHI, we will notify you no later than 60 days from the discovery of the breach. This notification will include a description of the incident, the PHI involved, and contact information for further inquiries.
Complaints and Contact Information
If you believe your privacy rights have been violated, you may contact us, the New Jersey Department of Health, or the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
